Mountin’ Hopes is in the process of reviewing the Rider Registration process to simplify it and to cut down on the number of forms. But, for now, please follow these steps for registering.
General Information
These forms need to be filled out for all new riders or if there is a change in the information (such as address) or a change in the rider’s condition/diagnosis. You do not need to fill them out before every riding session if nothing has changed.
If you are a new rider, please make sure that the forms are completed and returned to Mountin’ Hopes at least 2 weeks before the riding session starts. Click here to see the riding schedule.
A new rider must also schedule an ininitial evaluation with a Mountin’ Hopes Instructor at least two weeks prior to the session start date.
Once the forms are completed they should be mailed to Mountin’ Hopes at this address: Laura Nelson 292 Sun Meadow Lane, Pisgah Forest, NC 28768.
If you are an existing Mountin’ Hopes’ student with a change in information, you can bring that with you to the first lesson. Make sure the client/parent/guardian signs all documents.
NOTE: The scholarship application is at the bottom of the list. This form needs to be filled out at least once per year.

Forms that need to be Completed
1. Riders Medical History and Physicians Statement
- Make sure you leave yourself extra time because your child’s Doctor must complete & sign these forms
- Include the Doctor’s stamp/prescription sheet
- We know that this form takes awhile to fill out but it is very important for the instructor to understand specific items to focus on.
3. Rider Registration and Release
- Please attach a current photo of the rider
- Make sure you read & sign the Liability & photo release
4. Rider Authorization for Emergency Medical Treatment
- If you have insurance, please include a copy of your current insurance card
5. Riders Consent for Release of Information
- This form allows Mountin’ Hopes to release information concerning the rider’s experience to people who may use it to enrich the riders’ experience.
- Only fill this out if you want to apply for a scholarship OR if you already have a scholarship and your financial situation has changed.
- This needs to be filled out every year and if a scholarship is granted it will be used for each riding session.
- Include the first page of your Federal Income Tax form
- You will need to include one letter of reference from a non-family member
